Realised benefits
As with any system of this type “competence” plays a significant role. It is proven through the implementation of LOTO, gaps can be identified in risk assessments and safe operating procedures. As a result, Cenheard implemented trigger points into the LOTO process to identify any gaps.
Cenheard simplified the process for determining task LOTO requirements into six easy steps:
- Does LOTO apply for the task?
- Determine a baseline asset state category for the task.
- Identify the required isolation / lock-off points for the task.
- Determine the equipment required to implement LOTO.
- Install a localised LOTO board on each piece of equipment.
- Train, manage and optimise.
First steps
To start with, the first steps a series of definitions must be implemented into the procedure. This will keep the outcomes clear and concise and leave no room for interpretations.
From this Cenheard’s in-house multi-disciplined team of Health and safety professionals and machinery safety experts created a simple flow chart to follow for steps one and two. The first flow chart allowed the client to determine if the task required LOTO and the second flow chart determined what level of LOTO is required, this was identified via the “baseline asset state category”.
The quick and simple flow charts allowed the client to work through all the tasks for each machine and identify any gaps in internal documentation.
Key gap highlights our client found were as follows:
- Gaps in risk assessments.
- Gaps in safe operating procedures.
- Gaps in machinery knowledge regarding safety circuit functions.
Additional enhancements
After addressing the gaps, Cenheard identified the necessary isolation and lock-off points, along with the appropriate LOTO devices for each machine. These details were recorded in an isolation matrices that clearly defined each task and its LOTO requirements.
Working with one of our trusted partners, Cenheard assisted the client in designing and installing suitable LOTO boards beside each machine. Each board included the comprehensive isolation matrices and all relevant LOTO devices, providing personnel with a convenient “One-stop shop” for all task specific LOTO requirements.
This setup also enabled management to conduct straightforward audits to ensure safe working practices are being followed.
